Thank you for choosing TMJ & Sleep Therapy Centre of Chicago to receive care and treatment for your jaw and/or breathing issues. We welcome you as a new patient and congratulate you on taking charge of your disorder and seeking treatment. New Patient Questionnaire
Please note: You must call and speak with someone in the office BEFORE filling out the health history information.
Please ensure you bring the following to your appointment:
- Previous sleep test results.
- Any current sleep or TMJ appliances.
- Reports from previous doctor visits related to your TMJ/sleep issues.
- Any relevant imaging.
Important Notes:
- You are welcome to bring a family member with you.
- A $90 non-refundable reservation fee is required. Cancellations or rescheduling within 24 hours of the appointment will forfeit this fee, and another fee will be required for rescheduling.

What to Expect at Your First Appointment
During your initial visit, we will:
- Review your health history and specifics of your TMJ or sleep apnea.
- Conduct assessments including Joint Vibration Analysis (JVA), neurological tests, routine checks (temperature, blood pressure), a dental and muscular examination, and 3D radiographic imaging.
What to Bring:
- Previous sleep test results.
- Any TMJ/sleep-related appliances.
- Reports from past doctor visits regarding TMJ/sleep issues.
- Any relevant imaging.
- A second set of ears – bring a spouse/partner/sibling/parent/
child with you to the first appointment. You will hear a lot of information from Dr. Lynn and people often find it helpful to help talk it over and assimilate that information.
Outcomes of Your Visit:
- You will receive a preliminary diagnosis and treatment recommendations.
- We’ll discuss treatment costs and payment options.

- Our goal is to diagnose the root cause of your problem and provide immediate solutions to improve your condition from day one.
Please bring any additional documents that might be needed to confirm your diagnosis, such as recent sleep tests.
